Join the Practice
If you would like to register with the surgery, please check that you live in our catchment area below.
Due to overwhelming demand on our services, coupled with increasing difficulties travelling around Oxford, we can only accept registrations for patients living within our catchment area. If you live outside this area then you should register with a GP Surgery near you.
If you live in our catchment area and would like to register either an adult or a child aged 12 or over please complete our registration form which can be completed here. If you use this form for a child aged under 12 it will not be processed.
You can either bring photo ID and proof of address to the practice or email it to info.nbs@nhs.net
If you are registering a patient aged under 12 please go to the surgery to complete a paper form. If the child is aged under 5 we will require the child's vaccination record. If you have any queries please get in touch with the practice by calling 01865 240501 or email info.nbs@nhs.net
Live within boundary area?
Named GP
Every patient registered with 19 Beaumont Street Surgery has been allocated a named doctor to oversee their care. This doctor is known as the ‘named accountable GP’.
If you wish to find out your named GP, please contact the surgery or ask at your next appointment. Our doctors and nurses work as a team and, as your named GP will not be available at all times, you are welcome to continue to consult with an alternative doctor or nurse, particularly if your problem is urgent.
If you would prefer a particular doctor to be your named GP, please let us know.
