Access to your online medical record
Most patients will be able to view their medical records online by using the NHS App or Patient Access.
If you are having problems or would like access to your medical record and don’t have it, please complete our application for online access form.
You can use online access to Order repeat prescriptions, View upcoming appointments/cancel appointments and view your medical record, including test results, recent consultations, diagnoses, immunisations and documents.
Access to historic medical records
If you would like to have access to your historic notes, the process requires a trained staff member to look through your notes to ensure that harmful data is not released.
This can be quite time-consuming and means staff can't be helping other patients. Therefore, if possible, please request only specific information (certain dates or conditions).

If you would like to access your full medical record, you will need to submit a Subject Access Request.
